What do you need from me to file my claim? 

First: 
Patient information

We will verify that the patient's first and last name match their legal name, confirm the accuracy of their date of birth and legal sex, and ensure the mailing address and email address on file are up-to-date. This information is used to file the claim.

Second:
Insurance Information

To process the claim, we will also require the Primary Subscriber’s (Policy Holder’s) legal name, date of birth, and relationship to the patient. Additionally, a copy of both the front and back of the insurance card must be provided, as it is directly attached to the claim.

Next:
Visit Information

This is where our insurance expertise comes in! We will include all necessary visit information and submit the claim! 

Final:
Patient Notifications

Patient's will be notified via email once the claim has been submitted to Reimbursify. Patients will also have the opportunity to download the Reimbursify app to keep track of the progress of the claim.
